Core records
A useful preparation process usually starts with diagnosis summary, pathology, recent imaging reports, treatment history, medication list, and the specific question the patient wants organized.
Disease-specific additions
Lung, gastrointestinal, urologic, breast, head and neck, neurosurgical, lymphoma, and blood cancer cases may need different specialist records.
How to use this checklist
The checklist should help the coordinator assess readiness. It is not a clinical screening tool and should not be used to decide whether treatment or travel is suitable.
